A superbill is a detailed invoice that lists the services you’ve received from your therapist, along with the information your insurance company may require to process an out-of-network reimbursement claim.

A superbill includes:

• Your name and identifying information

• The provider’s name, credentials, and license number

• The provider’s address and contact information

• Dates of service and session details

• CPT (procedure) codes and diagnosis codes (if applicable)

• The amount you paid for each session

Why might you need one?
If your therapist is not in-network with your insurance company, you can submit a superbill directly to your insurer to request reimbursement. Payment from your insurance company is not guaranteed, and reimbursement amounts vary depending on your specific plan.

• CoorSelf PLLC will provide superbills upon request, but we do not bill insurance companies directly.

• You are responsible for understanding your out-of-network benefits and submitting your superbill to your insurance provider.
