Superbill Information
A superbill is a detailed invoice that lists the services you’ve received from your therapist, along with the information your insurance company may require to process an out-of-network reimbursement claim.
A superbill includes:
Your name and identifying information
The provider’s name, credentials, and license number
The provider’s address and contact information
Dates of service and session details
CPT (procedure) codes and diagnosis codes (if applicable)
The amount you paid for each session
Why might you need one?
If your therapist is not in-network with your insurance company, you can submit a superbill directly to your insurer to request reimbursement. Payment from your insurance company is not guaranteed, and reimbursement amounts vary depending on your specific plan.
Important:
CoorSelf PLLC will provide superbills upon request, but we do not bill insurance companies directly.
You are responsible for understanding your out-of-network benefits and submitting your superbill to your insurance provider.
